Extra Tuition Matauranga

1. I understand that photos/videos may be taken during the lesson and give permission for the photos to be used by Tauhara North No.2 Trust for promotional purposes ONLY.
2. I understand that I must give at least 24 hours notice tutors if my child/ren can not attend their scheduled lesson/s.
3. I understand that if my child/ren do no attend 3 scheduled lessons (without notice) a representative from Tauhara North No.2 Trust will contact me to put lessons on hold and/or cancel my child/rens lessons for the remainder of the term and I will have to re-apply for tuition for the next term.
4. I understand that my child/ren must have an attendance rate of 90% more with the provider to be considered for a Tuition Scholarship.
5. I consent to Tauhara North No.2 Trust to gain access to my child/rens personal data obtained by service providers to be used for attendance reports and progress reports.
